ABSTRACT

Hepatitis D virus (HDV) infection causes the most severe form of viral hepatitis with rapid progression to cirrhosis, hepatic decompensation, and hepatocellular carcinoma. Although discovered > 40 years ago, little attention has been paid to this pathogen from both scientific and public communities. However, effectively combating hepatitis D requires advanced scientific knowledge and joint efforts from multi-stakeholders. In this review, we emphasized the recent advances in HDV virology, epidemiology, clinical feature, treatment, and prevention. We not only highlighted the remaining challenges but also the opportunities that can move the field forward.

ABSTRACT

Objetivo: Analisar a incidência anual de hepatite D no Brasil e na região Norte, no período 2009-2018. Métodos: Estudo ecológico de casos de hepatite D notificados no Sistema de Informação de Agravos de Notificação (Sinan), analisados por sexo, idade e estados do Norte. Realizou-se análise de tendência temporal pelo método de Prais-Winsten, para estimar a variação percentual anual (VPA) das taxas de incidência. Resultados: No período, foram reportados 2.710 casos no Brasil, 74,5% na região Norte e 71,5% somente no Amazonas, Acre e Rondônia. Houve tendência de queda da VPA no país (-21,6% - IC95% -3,8;-36,2%), região Norte (-28,5% - IC95% -5,2;-46,1%,), Amazonas (-34,1% - IC95% -0,8;-56,2%) e Acre (-37,6% - IC95% -18,0;-52,6%). Verificou-se diminuição de casos nos estratos etários abaixo de 40 anos. Conclusão: Houve tendência de queda da incidência de hepatite D na Amazônia Ocidental, que impactou na incidência no país. Essa redução foi puxada pelos mais jovens, provavelmente resultado da vacinação para hepatite B.


Objetivo: Evaluar la incidencia anual de casos de hepatitis D en Brasil y en la Región Norte, entre 2009-2018. Métodos: En este estudio ecológico, se utilizaron casos notificados al Sistema Nacional de Agravamiento de de Enfermedades (Sinan), analizados por sexo, edad y estados del Norte. Se utilizó la regresión de Prais-Winsten para análisis de la tendencia temporal y el cálculo de la variación porcentual anual (VPA) de tasas de incidencia. Resultados: En el periodo, se notificaron 2.710 casos, siendo 74,5% en el Norte e 71,5% en Amazonas, Acre y Rondônia. Hubo tendencia a disminución de la VPA en Brasil (-21,6% - IC95% -3,8;-36,2%), región Norte (-28,5% - IC95% -5,2;-46,1%,), Amazonas (-34,1% - IC95% -0,8;-56,2%) y Acre (-37,6% - IC95% -18,0;-52,6%). Hubo disminución de casos entre los menores de 40 años. Conclusión: Hubo tendencia a la disminución de la incidencia en la Amazonía Occidental, impactando en la incidencia en Brasil. Esta baja está relacionada con los más jóvenes, probablemente debido a la vacunación contra la hepatitis B.


Objective: To analyze the annual incidence of hepatitis D cases in both Brazil and the Brazilian Northern region between 2009 and 2018. Methods: This was an ecological study of hepatitis cases notified on the Notifiable Health Conditions Information System (SINAN), analyzed by sex, age groups, and Northern region states. Temporal trend analysis was performed using the Prais-Winsten method to estimate incident rate annual percent change (APC). Results: In the period studied, 2,710 cases were reported in Brazil, 74.5% of them in the Northern region and 71.5% in Amazonas, Acre and Rondonia alone. APC showed a downward trend in Brazil as a whole (-21.6% - 95%CI -3.8;-36.2%), in the Northern region (-28.5% - 95%CI -5.2;-46.1%,), in Amazonas (-34.1% - 95%CI -0.8;-56.2%) and in Acre (-37.6% - 95%CI -18.0;-52.6%). Cases decreased in age groups below 40 years old. Conclusion: There was a downward trend in incidence in the Western Amazon, impacting incidence in Brazil as a whole. This fall was led by younger people, probably due to hepatitis B vaccination.

Resumen: Objetivo: Conocer el resultado de la vacunación contra la hepatitis B en las comunidades hiperendémicas Kandozi y Chapra de la Amazonia Peruana a partir de la prevalencia de infecciones por los virus de la hepatitis B (VHB) y Delta (VHD), ocho años después de iniciada la vacunación. Material y métodos: Se realizó un estudio transversal en 2 944 pobladores de 67 comunidades indígenas Kandozi y Chapra en abril de 2010. El tamizaje serológico para el antígeno de superficie del VHB (HBsAg), anticuerpos anti-HBc IgM e IgG, anticuerpos anti-HBs y anti-VHD se determinaron mediante pruebas de ELISA. Resultados: Las tasas de prevalencia del HBsAg, anti-HBc IgG, anti-HBs ≥10 mlUI/ml y anti-VHD fueron 2.3, 39.13, 50.95 y 2.11%, respectivamente. La prevalencia del HBsAg en niños <11 años fue cero. Entre los portadores del HBsAg, las tasas de prevalencia de sobreinfeccion por el VHD e infección aguda por el VHB fueron 2.11% (todos fueron >14 años) y 11.94%, respectivamente. Conclusiones: Estos hallazgos muestran la eliminación de portadores de VHB en niños <11 años, ocho años después de iniciada la vacunación contra el VHB.


Abstract: Objective: To determine the outcome of the vaccination against hepatitis, we determined the prevalence of hepatitis B virus (HBV) and hepatitis D virus (HDV) infections, eight years after introduction of the vaccination. Materials and methods: A cross-sectional study was performed in 2 944 participants of 67 Kandozi and Chapra indigenous peoples in April 2010. Serological screening for hepatitis B surface antigen (HBsAg), antibody anti-HBc IgM and IgG, antibody anti-HBs and anti-HDV were determined by ELISA tests. Results: The prevalence rates of HBsAg, anti-HBc total, anti-HBs ≥10 mlUI/ml and anti-HDV were 2.3, 39.13, 50.95 and 2.11%, respectively. The prevalence rate of HBsAg in children <11 years was 0%. Among carriers of HBsAg, the prevalence rates of HDV and acute HBV infections were 2.11% (all were >14 years) and 11.94%, respectively. HBsAg and anti-HBc total were associated with individuals ≥10 years (p<0.001). Conclusions: These findings show the elimination of HBV carriers in children <11 years, eight years following introduction of the vaccination against HBV.

Abstract INTRODUCTION: Brazil's western Amazon basin has the highest prevalence of hepatitis B virus (HBV) infection in the country. Coinfection with hepatitis D virus (HDV) is also endemic. To estimate the prevalence of HBV and HDV markers in a population inhabiting the northwest portion of Mato Grosso state in the western Amazon. METHODS: We performed a cross-sectional study of the seroprevalence of antibodies against HBV core antigen (anti-HBc) in the Três Fronteiras District northwest of Mato Grosso. Anti-HBc-positive subjects were tested for HBV surface antigen (HBsAg). Those positive for this marker were tested for HDV antibodies. Anti-HBc-negative participants were tested for anti-HBsAg. All tests were performed by EIA. RESULTS: A total of 623 individuals in the community were assessed; the majority (67.6%) were male, with a mean age of 30.8 ± 15.4 years. Two hundred and fourteen individuals (34.3%) were anti-HBc-positive, and 47 (7.5%) were HBsAg carriers. Only one individual was anti-HDV-positive. Among the 409 individuals without HBV infection, 18.3% were anti-HBsAg-positive. There was no association between HBV infection and known risk factors. CONCLUSIONS: The study area had intermediate-to-high endemicity for HBV infection, but a low prevalence of HDV. Our serological results suggesting low vaccination-induced protection indicate a need for reinforced immunization programs in the populations of northwest Mato Grosso.

ABSTRACT Objective: to analyze clinical, serological, biochemical and hematological aspects in patients infected with the hepatitis B (HBV) and Delta (HDV) viruses. Method: cross-sectional, descriptive and retrospective study, performed with patients chronically infected with HBV and superinfected with HDV. Results: among the 112 patients selected, 74% were monoinfected with HBV (Group HBV) and 26% were superinfected with HDV (Group HBV+HDV). There was no difference in gender distribution. The average age was 36 years with standard deviation of ±12 years. The symptoms and signs presented a higher proportion in Group HBV+HDV (p=0.001). In both groups, most patients had non-reactive AgHBe. The records of biochemical and hematologic changes showed highest proportion in Group VHB+VHD Group (p<0.05). Conclusion: the study found that patients were in clinical stages of the disease different from those in the initial examination for monitoring their chronic condition. The clinical profile suggests greater severity of liver disease among the patients superinfected with HDV.


RESUMEN Objetivo: Analizar los aspectos clínicos, serológicos, bioquímicos y hematológicos de pacientes infectados por el virus de las hepatitis B (VHB) y Delta (VHD). Método: Se trata de un estudio transversal, descriptivo, retrospectivo, realizado entre pacientes crónicos infectados de VHB y sobre infectados de VHD. Resultados: Entre los 112 pacientes seleccionados, el 74% estaba mono infectado por VHB (Grupo VHB) y el 26%, sobre infectado por VHD (Grupo VHB+VHD). No se encontró diferencia en la distribución por género. La edad promedio era 36 años, con desviación típica de ±12 años. Los síntomas y signos sobresalían en mayor proporción en el grupo VHB+VHD (p=0,001). Para ambos grupos, la mayoría de los pacientes estaba con AgHBe no reactivo. El registro de alteraciones bioquímicas y hematológicas atribuyó proporción más grande al grupo VHB+VHD (p<0,05). Conclusión: El estudio demostró que los pacientes, en la consulta inicial para el seguimiento de la condición crónica, estaban en diferentes estadios clínicos de la enfermedad. El perfil clínico sugiere que la gravedad de la enfermedad hepática es mayor entre pacientes sobre infectados de VHD.

Para ambos os grupos, a maioria dos pacientes estava com AgHBe não reagente. O registro de alterações bioquímicas e hematológicas apresentou maior proporção no grupo VHB+VHD (p<0,05). Conclusão: O estudo revelou que os pacientes estavam em diferentes estágios clínicos da doença na consulta inicial para acompanhamento de condição crônica. O perfil clínico sugere maior gravidade da doença hepática entre os pacientes superinfectados por VHD.

Abstract Hepatitis delta virus (HDV) has been associated with acute or chronic hepatitis in Latin America, but there is no prevalence study covering South American countries. This meta-analysis aimed to estimate anti-HDV prevalence through a systematic review of published articles in English, Portuguese and Spanish until December 2017. Searches were conducted in Health Virtual Library, Capes, Lilacs, PubMed, and SciELO, according to defined criteria regarding participant selection and geographical setting. Study quality was assessed using the GRADE guidelines. Pooled anti-HDV prevalence was calculated using the DerSimonian-Laird random-effects model with Freeman-Tukey double arcsine transformation. Out of the 405 identified articles, only 31 met the eligibility criteria for inclusion in the meta-analysis. In South America, pooled anti-HDV prevalence among hepatitis B virus carriers was 22.37% (95% confidence interval: 13.72-32.26), though it appeared less frequently in some countries and populations, according to the data collection date. The findings indicated significant successive reductions in anti-HDV prevalence over thirty years. However, there was a scarcity of HDV epidemiological studies outside the Amazon Basin, notably in the Southwest continent and absence of target population standardization. There was a high HDV prevalence in South American countries, despite differences in methodological characteristics and outcomes, highlighting a drastic decline in the last decades. Future studies should identify HDV prevalence estimates in other regions of the continent and identify risk factors.

ABSTRACT

INTRODUCTION: A decline in hepatitis D virus (HDV) occurrence was described in Europe and Asia. We estimated HDV prevalence in the Brazilian Amazon following hepatitis B vaccination. METHODS: This is a cross-sectional survey of HDV measured by total antibodies to HDV (anti-HD T). RESULTS: HDV prevalence was 41.9% whiting HBsAg carries and was associated with age (PR = 1.96; 95% CI 1.12-3.42; p = 0.01), hepatitis B virus (HBV) infection (PR = 4.38; 95% CI 3.12-6.13; p < 0.001), and clinical hepatitis (PR =1.44; 95% CI 1.03-2.00; p = 0.03). Risk factors were related to HDV biology, clinical or demographic aspects such as underlying HBV infection, clinical hepatitis and age. CONCLUSIONS: Our study demonstrated that HDV infection continues to be an important health issue in the Brazilian Amazon and that the implementation of the HBV vaccination in rural Lábrea had little or no impact on the spread of HDV. This shows that HDV has not yet disappeared from HBV hyperendemic areas and reminding that it is far from being a vanishing disease in the Amazon basin.


INTRODUÇÃO: É descrito declínio na ocorrência do vírus da hepatite D (VHD) na Europa e Ásia. Estimamos a prevalência de infecção pelo VHD na Amazônia Ocidental, após a introdução da vacinação contra hepatite B. MÉTODOS: Este é um estudo de corte transversal da prevalência do VHD medido pela ocorrência de anticorpos totais (anti-HD T). RESULTADOS: A prevalência do VHD encontrada foi 41,9% entre os portadores do HBsAg, e esteve associado à idade (RP = 1,96; IC 95% 1,12-3,42; p = 0,01), infecção pelo HBV (RP = 4,38; IC 95% 3,12-6,13; p < 0,001) e história clínica de hepatite (RP =1,44; IC 95% 1,03-2,00; p = 0,03). Fatores de risco mostraram-se associados à biologia do HDV, aspectos clínicos e demográficos como infecção prévia pelo VHB e idade. CONCLUSÕES: O estudo demonstra que a infecção pelo VHD continua sendo um importante problema de saúde pública na região, e que a implantação da vacinação contra o VHB na área rural de Lábrea teve um impacto pouco significativo no controle do VHD, percebe-se que este ainda não desapareceu de áreas hiperendêmicas do VHB, e está longe de poder ser classificado como uma doença em declínio na bacia Amazônica.

JUSTIFICATIVA E OBJETIVOS: As hepatites causadas pelos vírus da hepatite B (VHB), vírus da hepatite C (VHC) e vírus da hepatite (VHD) têm como aspecto comum a transmissão por via parenteral e a possibilidade de cronificação. Revisar os aspectos clínico-epidemiológicos, diagnósticos, terapêuticos e profiláticos das infecções virais por tais agentes é o escopo do presente artigo. Realizou-se pesquisa bibliográfica nas bases de dados Scielo e Pubmed empregando-se os descritores hepatite B (hepatitis B); hepatite C (hepatitis C); hepatite D (hepatitis D) e hepatite G (hepatitis G), assim como livros texto, consensos e diretrizes relacionadas ao tema.CONTEÚDO: As formas agudas das hepatites B, C e D são usualmente benignas, podendo, sem embargo, ocorrerem quadros de hepatite fulminante. Em situações nas quais o sistema imunológico não é capaz de depurar o VHB e/ou VHC, há cronificação da infecção, com risco de desenvolvimento de cirrose e consequente insuficiência hepática crônica, bem como carcinoma hepatocelular. As hepatites B e D são imunopreveníveis, graças à vacina parao vírus B, mas, até o momento, não há imunoprofilaxia disponível para o vírus C.CONCLUSÃO: As hepatites pelos VHB e VHC constituem importantes desafios para a medicina atual, especialmente pela prevalência das infecções no planeta e pelo risco de desenvolvimento das complicações crônicas. Neste contexto, destaque-se a importância da avaliação diagnóstica, da instituição da terapêutica adequada e do emprego das medidas preventivas para tais infecções, elementos que devem ser solidamente conhecidas pelo clínico.


BACKGROUND AND OBJECTIVES: Hepatitis caused by hepatitis B virus (HBV), hepatitis C virus (HCV) and hepatitis D virus (HDV) have in common the transmission by parenteral route and the possibility of chronification. Reviewing the clinic, epidemiology, diagnose, therapeutics and prophylaxis of viral infections by these agents is the scope of this work. Bibliographic research was conducted both at databases Scielo and Pubmed through the chosen descriptors: hepatitis B, hepatitis C, hepatitis D and hepatitis G, and text books, consensus and guidelines related to the subject.CONTENTS: The acute viral B, C and D hepatitis are usually benign, though acute liver failure, fulminant hepatitis, may occur. In the cases when the immune system is unable to debug HBV and HCV the infection becomes chronic, cirrhosis with consequent chronic liver insufficiency and hepatocellular carcinomamay develop. HBV and HDV are immunopreventable, thanks to the hepatitis B virus vaccine, but at this point there's no immunoprophylaxis available for hepatitis C virus. CONCLUSION: HBV and HCV hepatitis are great challenges for medicine, particularly due to the prevalence of infections worldwide and the risk of chronic complications. In this context, diagnostic evaluation, adequate therapeutic care, and preventive measures must be soundly known by the physician.

ABSTRACT

Viral Hepatitis B, C and D are a serious public health problem in Brazil and other South American countries, mainly in the Amazonian region. Despite the paucity of clinical and epidemiological studies, a high prevalence of Hepatitis viruses has often been described in this area. Genotype F of Hepatitis B and Genotype III of Hepatitis D have been found to be quite prevalent in this area and preliminary studies have implicated both genotypes in carcinogenesis and peculiar pathogenic liver mechanisms. Initial epidemiological studies have further demonstrated a high prevalence of Hepatitis C in the western Brazilian Amazon. The geographic, cultural, ethnic and environmental aspects of this region may favor hepatotropic virus dissemination, as well as rendering difficult the implementation of governmental programs in the treatment of patients and prevention of disease dissemination.

ABSTRACT

El virus de la hepatitis B es un virus DNA altamente infeccioso cuyo estado de portador crónico conlleva a la cirrosis y el cáncer de hígado y esto se da en plazos mas cortos si está asociado al virus D como ocurre en el Perú. Nuestro país tiene en promedio una endemicidad intermedia, aunque hay áreas de alta, mediada y baja endemicidad; sin embargo la migración de personas entre estas áreas, está dispersando la infección. Debe destacarse en el pais la transmisión horizontal en niños de áreas endémicas y la baja transmisión vertical, lo que es una ventaja para proteger tempranamente con la vacuna. La vacuna disponible contra HBV es eficaz y segura, y siendo la vacunación una de las intervenciones más costo efectivas, se han desarrollado programas de vacunación piloto en el país, con un significativo impacto sobre la tasa de infección; por lo que amerita ampliar la población objetivo a inmunizar, como son los adolescentes y jóvenes y grupos de riesgo de manera que se acorten los periodos para eliminar la infección por HBV y por tanto sus secuelas crónicas. Hay avances en la prevención, pero aun queda que hacer con los portadores crónicos. Habiendo perspectivas terapéuticas tanto para los HBeAg positivos y HBeAg negativos, será pertinente plantear programas de tratamiento y de otro lado evaluar todas las intervenciones efectuadas. Estos son los temas que se incluyen en esta revisión.


Hepatitis B virus is a highly infectious DNA virus, and the chronic carrier status may lead to cirrhosis and liver cancer. These conditions may supervene in a short time period if there is coexistence of both hepatitis B and D viruses, as it usually occurs in Peru. Our country has an intermediate endemicity, although there are areas with high, medium and low endemicity; however, migration is spreading these infections. It is important to point out that there is horizontal transmission in children from endemic areas and a low level of vertical transmission, which is an advantage in order to achieve early protection using the hepatitis B vaccine. This vaccine HBV is efficacious and safe, and vaccination is still one of the most cost-effective interventions. Pilot hepatitis B vaccination programs have been implemented in Peru, achieving a significant impact on the infection rates; so it is advisable to expand the target population, particularly teenagers and other risk groups, so that the time period to eliminate hepatitis B infection may be shortened, as well as its chronic sequels. There have been some advances in prevention, but we still have to take care of chronic carriers. Knowing that there are therapy strategies available for subjects positive and negative for HBeAg, it is advisable to propose therapy programs and to assess all interventions performed. These are the topics included in this review.

El Perú está catalogado como un país de mediana endemicidad para la infección por la hepatitis B (HBV), sin embargo, esta enfermedad se está dispersando en el país por la intensa migración de áreas de alta endemicidad a zonas de baja que endemicidad. Además de los mecanismos clásicos de transmisión de la infección como la vía parenteral y la vía sexual, debe destacarse la transmisión horizontal sobre todo en población infantil de áreas hiperendémicas y la limitada ocurrencia de transmisión vertical a diferencia de Asia. Desde que se cuenta con una vacuna segura y eficaz contra la HBV, se ha desarrollado programas de vacunación piloto en el país, luego de lo cual se ha generalizado la inmunización de la población infantil primero de áreas endémicas y luego a todo el país, según la prioridad y disponibilidad de vacunas. Dada la dispersión de la infección, y para lograr un efectivo control del problema en personas susceptibles, en un menor tiempo, es necesaria la inmunización contra HBV de adolescentes, jóvenes y grupos de riesgo, intervención que es una de las más costo-efectivas en salud pública, mereciendo en el mediano y largo plazo la evaluación de su real impacto. Estos temas son los que se desarrollan en la presente revisión.


Peru is catalogued as a middle-endemic country for hepatitis B infection (HVB), however, this disease is spreading in the country, by intense migration of highly endemic to low endemic areas. In addition to the classical mechanisms of transmission as the parenteral and sexually, it should be noted the horizontal transmission among children especially in hiperendemic areas and limited occurrence of vertical transmission. Since there is a safe and effective vaccine against HBV, the Peruvian Ministry of Health has developed a pilot vaccination programs in the country, after which has been widespread immunization to children first and then in endemic areas throughout the country, according the priority and availability of vaccines. Given the spread of infection, and to achieve an effective control of the problem in susceptible people, in less time is needed against HBV immunization of adolescents, young people and risk groups, that intervention is one of the most cost-effective public health, is needed in the medium and long term the evaluation of its real impact. These issues were development in this review.

Hepatitis D virus [HDV] infection is considered to be high in the Mediterranean basin. Data regarding this infection in Libyan population are scarce. The aim of the present study was to determine the seroprevalence of anti-HDV antibodies in hepatitis B virus [HBV] infected patients in Benghazi. The study population comprised one hundred thirty-eight patients with chronic HBV infection who were attending the liver clinic, Aljamahiriya Teaching Hospital, Benghazi. The diagnosis of HBV infection in those patients was based on repeatedly positive HBsAg in the serum with other laboratory and / or radiologic features of liver disease. All patients were tested for the presence of Anti-HDV IgM antibodies and Anti-HDV IgG antibodies using ELISA techniques. At the same time the level of HBV-DNA was measured using polymerase chain reaction [PCR] techniques for both Anti-HDV antibodies positive and negative patients. Fifteen patients were positive for anti-HDV IgG antibodies [10.8%] and none was positive for Anti-HDV IgM antibodies [0%]. The HBV-DNA levels were higher in HBV patients with Anti-HDV antibodies than in patients positive for Anti:HDV Anti- HDV antibodies [P=0.01]. HDV infection does not appear to be commonly prevalent in Libyan patients with HBV. In comparison to data from previous results from Mediterranean area, the seroprevalence of HDV in Libyan patients who were attending the liver clinic, Benghazi is considered to be intermediate

Comorbidities in human immunodeficiency virus infection are of great interest due to their association with unfavorable outcomes and failure of antiretroviral therapy. This study evaluated the prevalence of coinfection by human immunodeficiency virus and viral hepatitis in an endemic area for hepatitis B in the Western Amazon basin. Serological markers for hepatitis B virus, hepatitis C virus and hepatitis D virus were tested in a consecutive sample of all patients referred for treatment of human immunodeficiency virus or acquired immunodeficiency syndrome. The variables sex, age, origin and exposure category were obtained from medical records and from the sexually transmitted diseases and acquired immunodeficiency syndrome surveillance database. Among 704 subjects, the prevalence of chronic hepatitis B carriage was 6.4 percent and past infection 40.2 percent. The presence of hepatitis B was associated with birth in hyperendemic areas of the Amazon basin, male sex and illegal drug use. The overall prevalence of hepatitis C was 5 percent and was associated with illegal drug use. The prevalence of hepatitis B and C among human immunodeficiency virus or acquired immunodeficiency syndrome patients in the Western Amazon basin was lower than seen elsewhere and is probably associated with the local epidemiology of these viruses and the degree of overlap of their shared risk factors. An opportunity presents itself to evaluate the prevention of hepatitis C through harm reduction policies and hepatitis B through vaccination programs among human immunodeficiency virus or acquired immunodeficiency syndrome patients.


Co-morbidades na infecção pelo vírus da imunodeficiência humana são de grande interesse devido à associação com desfechos desfavoráveis e falhas na terapia anti-retroviral. Este estudo avalia a prevalência de co-infecção entre o vírus da imunodeficiência humana e hepatites virais, em uma área endêmica de hepatite B, na Amazônia Ocidental. Marcadores sorológicos para o Vírus da hepatite B, Vírus da hepatite C e vírus da hepatite D foram testados em uma amostra de pacientes referenciado para o tratamento em ambulatório para pacientes com infecção pelo vírus da imunodeficiência humana ou síndrome da imunodeficiência adquirida. As variáveis: sexo, idade, origem e categoria de exposição, foram obtidas dos prontuários médicos e do banco de dados da Coordenação Estadual de Doenças sexualmente transmissíveis. Entre os 704 indivíduos incluídos, a prevalência de portador crônico do vírus da hepatite B foi 6,4 por cento e de infecção passada 40,2 por cento. O vírus da hepatite B esteve associado com o local de nascimento em áreas hiperendêmicas da Amazônia, sexo masculino e uso de drogas ilícitas. A prevalência de hepatite C foi 5 por cento, estando associada com uso de drogas ilícitas. A prevalência dos vírus da hepatite B e C, entre indivíduos vivendo com o vírus da imunodeficiência humana e síndrome da imunodeficiência adquirida, na Amazônia ocidental, foi mais baixa que as observadas em estudos com populações de outras regiões. Provavelmente, estes resultados estão associados à epidemiologia local destes vírus, e ao grau de superposição dos fatores de risco associados à infecção na população estudada. O estudo apresenta oportunidade de avaliar a prevenção da hepatite C e B, através de medidas de redução de danos e programa de vacinação em indivíduos vivendo com vírus da imunodeficiência humana e síndrome da imunodeficiência adquirida.

Hepatitis B virus (HBV) is the most important causative agent of blood borne hepatitis in humans. Hepatitis D Virus (HDV) infection occurs either as a coinfection or superinfection in HBV carriers. Hepatitis C virus (HCV) is the major cause of transfusion non-A, non-B hepatitis and continues to be a major cause of human liver disease throughout the world. The present study was conducted on 70 clinically diagnosed cases of viral hepatitis to study the prevalence of parenterally transmitted viral hepatitis. The serum samples were tested for HBsAg, HBeAg, IgM anti-HBc, anti-HBe, anti-HCV and anti-HDV using separate ELISA kits. Of the 70 serum samples tested, 28 (40%) were positive for HBsAg out of which 3 (4.28%) were positive for HBeAg also. Five (7.1%) of the HBsAg positive cases tested positive for IgM anti-HBc also. HBsAg alone was found in 17 (24.28%) cases. The prevalence of anti-HCV was 3 (4.28%) in 70 cases. Thus early screening of clinically diagnosed cases of viral hepatitis is essential for establishing diagnosis and treatment to prevent long term sequelae.

Entre populações autóctones da América, estudos relatam altos índices de infecção e doença pelos vírus das hepatites B e D. Esta é uma revisão do que já foi descrito entre indígenas da Amazônia brasileira. Em alguns grupos a prevalência do AgHBs é muito baixa, enquanto que outros da mesma região, apresentam padrão de elevada endemicidade, presente inclusive entre menores de 10 anos. O VHD só foi encontrado entre etnias no estado do Amazonas. É descrito a importância da transmissão horizontal familiar, e do contato sexual entre adultos jovens. Fatores socioculturais, genéticos, ecológicos, e a formação histórica desses povos, são apontados como determinantes deste padrão. Entretanto, a origem do VHB e VHD na Amazônia é ainda obscura. Populações indígenas com sua memória genética são, na verdade, o experimento ao vivo, o que demanda investigação abrangente, avaliando a influência dos aspectos históricos, ecológicos, médicos e antropológicos envolvidos, utilizando inclusive técnicas modernas de biologia molecular.


Several studies describe very high prevalence rates of infection and disease of hepatitis B and D within Native American population. This is a review of what has been described among Amerindians of Brazilian Amazon. Some groups show low prevalence rates of HBsAg, whereas, others of the same region reveal high endemic pattern, even among individuals less then 10 years of age. HDV is only found in groups of Amazonas province. Transmission may occur by interfamilial dissemination or sexual contact among young adults. Socio-cultural, genetic, and ecological factors are described as determinants of this unique pattern. Nevertheless, the origin of these two viruses is yet to be disclosed. Amerindians population and their genetic memory are a live experiment, which demands a broad investigation, weighting with modern tools, as molecular biology, the influence of historical, genetic, medical and anthropological factors.

ABSTRACT

A hepatite B é a principal causa de doença hepática na Amazônia, sendo um de seus maiores problemas de saúde pública. A partir dos anos 70, intensificou-se a migração para o sul da Amazônia. No norte do Estado de Mato Grosso, Brasil, foram identificados surtos de hepatite B comunitária e alta prevalência de seus marcadores entre os migrantes após meses da chegada. Análise de subtipos do antígeno de superfície do virus sugere que os migrantes trouxeram o agente infeccioso de suas regiões de origem. Fatores ambientais e comportamentais provavelmente facilitaram a rápida disseminação do vírus da hepatite B nessas comunidades. Dados mais recentes demonstram que a manutenção de vacinação e vigilância nas regiões mais acometidas está diminuindo a incidência da infecção. O aumento do número de casos de hepatite delta entre os portadores do vírus B no norte do Estado de Mato Grosso começa a ser detectado, provavelmente resultante do maior contato com os Estados vizinhos, que têm alta prevalência de hepatite delta.


Hepatitis B is the main cause of hepatic disease in the Amazon and a biggest problem for its public health. Outbreaks and high prevalence of hepatitis B markers have been reported among immigrants recently arrived in northmost part of the Brazilian State of Mato Grosso. The hepatitis B surface antigen subtypes analysis suggests that the immigrants brought the virus themselves. Likely environmental and behavioral factors have facilitated the hepatitis B spread among these immigrant communities. Recent data show that surveillance and vaccination of immigrants have decreased the incidence of hepatitis B in this region. The increasing number of hepatitis delta cases among hepatitis B carriers from the Northmost part of Mato Grosso State has been detected as a consequence of a increasing contact with neighbor States that present high prevalence of this coinfection.

Drug addicts frequently have liver diseases for different reasons: alcohol abuse, the drugs themselves, but more often hepatitis B and C infections. AIDS is common in this population as well and could also affect the liver directly or in the form of hepatocellular or biliary damage. We conducted this study to determine the prevalence of liver diseases, alcoholism, hepatitis B and C infections, and HIV positivity in this population. We studied a cohort of 137 persons, all with a history of drug abuse, and investigated the quantity of alcohol intake, the kind of drug used, and he routes of drug administration. RESULTS: We found liver disease in 33.6%. The prevalence of alcoholism was 65.4%, of HCV 67.3%, and of HBV 17.3%. HDV was undetectable, whereas we found HIV at a frequency of 17.3%. HCV RNA was detected in 85.4% of HCV. The drug most often used was cocaine at 90.4%, followed by marihuana at 88.3%; LSD use occurred in 17.5%. We found parenteral drug use in 43.1%. We performed 22 liver biopsies, 21 associated with HCV, and detected histological changes consistent with chronic hepatitis in 17, with cirrhosis in 4, and with hepatocellularcarcinoma in 1.

O vírus da hepatite D (VHD), também chamado de vírus delta, é um pequeno vírus contendo RNA circular. O VHD causa infecçäo, quando há coinfecçäo com o vírus da hepatite B (VHB) em indivíduos normais ou superinfecçäo em portadores crônicos do VHB. Três genótipos já foram clonados e seqüenciados. A infecçäo apresenta distribuiçäo mundial, sendo a regiäo ocidental da Amazônia brasileira considerada área de alta endemicidade. Estima-se que 18 milhöes de pessoas encontram-se infectadas pelo vírus entre os 350 milhöes de portadores crônicos do VHB no mundo. As vias de transmissäo do VHD e os fatores de risco mostram-se similares aos da infecçäo pelo VHB. O diagnóstico se faz pela identificaçäo imuno-histológica do HDAg no fígado e pelo encontro das fraçöes IgM e IgG anti-HD no soro por radioimunoensaio ou ELISA. O curso clínico da infecçäo pelo VHD mostra-se variável. Os pacientes podem apresentar formas fulminantes de hepatite. As formas crônicas associam-se a achados histopatológicos graves no fígado, com curso rápido e progressivo, evoluindo para cirrose, insuficiência hepática e morte. O interferon alfa constitui a única opçäo terapêutica com algum efeito benéfico no tratamento da hepatite. O transplante hepático encontra indicaçäo nos casos terminais de cirrose. A profilaxia indireta da infecçäo pelo VHD tornou-se possível com o advento da vacina contra o vírus da hepatite B

ABSTRACT

HBV and HDV infections are a major health problem in the Amazon. This study evaluates the prevalence of these viruses within Indians groups, measuring the frequency of serological markers. The prevalence of past infection was 54.5% and HBsAg was found in 9.7%. An important variation of those markers was detected between villages, inclusively within the same ethnic group. None showed evidence of an acute infection. All HBeAg reactive were Apurinã, living in the same village and three of them from the same family. The prevalence of anti-HDT was 13.4% in HBsAg positive individuals. The authors observed high prevalence of HBV and HDV infection with the following pattern: low degree of infectivity, marked transmission in early ages, intra-familial dissemination, and lack of importance of vertical transmission. However, the results suggest that these viruses were not yet introduced effectively in some of the studied groups.


A infecção pelo VHB e VHD são importantes problemas de saúde na Amazônia. Este estudo avalia a prevalência da infecção por esses agentes em sete grupos indígenas do Estado do Amazonas. A taxa de infecção passada pelo VHB encontrada foi de 54,5% e a de portadores do AgHBs de 9,7%. Observa-se variação importante destes marcadores entre as aldeias, inclusive da mesma etnia. Não evidenciamos marcador de infecção aguda, os quatro AgHBe reativos eram todos Apurinã, da mesma aldeia, e três da mesma família. O VHD foi encontrado em 13,4% dos AgHBs reativos. O padrão de infecção pelo VHB e VHD encontrado possui as seguintes características: endemicidade elevada, baixo potencial de infectividade, transmissão marcada em idade precoce, provável transmissão familiar, e pouca importância da transmissão vertical. Entretanto, também sugere que esses vírus não tenham sido ainda introduzidos efetivamente em algumas das etnias estudadas

Background: Sanitary and socioeconomic changes and the identification of new causative virus, have changed the epidemiology of hepatitis in Chile. Aim: To study the natural history of acute hepatitis caused by virus A, E and non A-E in Chilean adults. Patients and methods: A special study protocol was followed for patients with a clinical picture of acute hepatitis. Anti HAV IgM, anti HBc IgM, anti HEV IgG and IgM and Anti HCV antibodies were determined by ELISA. Results: Fifty nine patients (30 male), aged 15 to 58 years old were studied. Eighty nine percent had jaundice and 50 to 70 percent had malaise and abdominal pain. Virus A was positive in 80 percent, virus E in 7 percent. In 14 percent of patients, all viral markers were negative. The evolution was typical in 78 percent, biphasic in 14 percent and cholestatic in 5 percent. One patient had a prolonged and one a fulminant course. Mean ALT was 1148 U/l and mean total bilirubin was 5.5 mg/dl. Seventy three percent of cases occurred during early winter and spring and 27 percent during summer and early autumm. Conclusions: The main etiology of acute viral hepatitis in Chile is virus A and most cases occur during the rainy season. Clinical features of hepatitis non A-E are similar to enteral transmission forms
